29 CFR 1926.102(a)(2): Eye and face protection required by this Part did not meet the requirements specified in American National Standards Institute, Z87.1-1968, Practice for Occupational and Educational Eye and Face Protection:  (a)  Roof at 741 Tarpley Rd. Kennesaw, GA: On or about July 20, 2016, employees installing roofing shingles were exposed to eye injuries while operating Hitachi NV 45AB2 coil nailers,  in that; no eye protection was in use.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  Roof at 741 Tarpley Rd. Kennesaw, GA: On or about July 20, 2016, the employer exposed employee to a lower level fall of approximately 14-feet, in that; no fall protection was in use.   Diego Saldivar Rivera was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1053518, citation number 2, item number 1 and was affirmed as a final order on 07/15/2015, with respect to a workplace located at 2174 Belmont Trace, Lot # 38, Powder Springs, Georgia.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) Roof at 741 Tarpley Rd. Kennesaw, GA: On or about July 20, 2016, where employees were accessing the roof, the employer did not ensure the ladder's  side rails were extended at least 3-feet above the landing surface.   Diego Saldivar Rivera was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1) which was contained in OSHA inspection number 1053518, citation number 2, item number 2 and was affirmed as a final order on 07/15/2015, with respect to a workplace located at 2174 Belmont Trace, Lot # 38, Powder Springs, Georgia.
