On or about at 7/21/16 located at 5425 West Point Dr, Cumming, GA: Employees were framing and installing the 2nd floor, roof rafters on a house that was under construction while working at the edges of the staircase floor hole opening, the employer did not ensure that guardrails or floor hole coverings was used by the employees while exposed to fall hazards of approximately up to 10.4- feet.

Cilo A. Mozo was previously cited for a violation of this occupational safety and health standard or its equivalent standard 192. 501 (b)(13), which was contained in OSHA inspection number 1047244, citation number 1, item number 3, and was affirmed as a final order on April 20/2015, with respect to a workplace located at 5290 Northview Lake., Cumming, GA.
