29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: a. On or about July 21, 2016, at the Palm Coast Chase Bank jobsite, in the 8.5 feet deep trench, employees installing drainage piping were exposed to a cave-in hazard in that they were not protected by an adequate protective system.
