29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employees: (a) On and at times before July 19, 2016, at the facility located at 707 W HWY 60, Monett. MO., employees were exposed to serious injuries, in that the employer had not selected and enforced the use of necessary personal protective equipment needed to protect the hands and fingers of affected employees handling sharp metal materials and or machinery/equipment. As a result employees have received numerous recordable injuries involving cuts, lacerations, and a recent thumb amputation. These incidents could have been prevented if proper hand protection was being provided and used during the tasks involved. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycles: (a) On July 19, 2016, at the facility located at 707 W HWY 60 - Monett, MO., employees were exposed to serious injuries to their hands and or fingers, in that the Point of Operation was not guarded to prevent employees from the danger zone of the operating cycles in the following instance: (1) The Accu-Press Brake Press # 71714 located in the SE section of Brake Metal Area - did not have any Safe Guards or Devices to protect the Point of Operation. (2) The Delta Rockwell Band Saw SN#1387258, in the SE section of Brake Metal Area - 2-1/2 inches of the unused portion of the cutting blade was left unguarded while cutting metal approx. 1/16th of inch thick. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
