29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: Worksite located at 298 E. Cedar St., Granby, Missouri- The employer had installed fork extenders, but had failed to contact the manufacturer in order to obtain written approval to perform the modification. The modification changes the lifting dynamics of the lift and changes the capacities of the lifts.

29 CFR 1910.178(p)(1):Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition: (a). Worksite located at 298 E. Cedar St., Granby, Missouri- Employees are exposed to struck-by or caught-in between hazards in that the brakes on the Hyster 80 forklift were not engaging adequately in order to stop the vehicle in a safe manner. (b). Worksite located at 298 Cedar St., Granby, Missouri- The Hyster 80 and Caterpillar DPL 402 did not have audible back-up alarms to notify any employee working around the forklift.
