29 CFR 1926.552(c)(15): The certification record for the inspection and testing of personnel hoist did not include the date the inspection and test of all functions and safety devices were performed; and a serial number, or other identifier, for the hoist:    a)  On or about July 19, 2016 - at the above addressed jobsite, the employer did not ensure that the inspection records of the Skip Hoists of the site included serial numbers or other identifiers for the Skip Hoists A, B, and C used at the site, to aid in proper certification of testing of all functions and safety devices.
