(a) Abraham, WV on Interstate 64: On or about July 15, 2016 and times prior, a 24ft. Werner extension ladder used for entering and exiting the excavation was located on the excavation wall outside of the provided trench box approximately 15ft. from the protected area.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary.    (a) Abraham, WV on Interstate 64: On or about July 15, 2016, and times prior loose rocks were located on the edge of an excavation at the access point, excavated material and a Komatsu back hoe were placed on the edge of the excavation where employees were performing boring activities.      The Mike Enyart and Sons, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, Protection of Employees from loose rock and soil, which was contained in OSHA inspection number 315225110, citation number 1, item number 4a and was affirmed as a final order on December 13, 2011, with respect to a workplace located on Washington Street in Paden City, WV.

(a) Abraham, WV on Interstate 64:  On or about July 15, 2016 and times prior, employees working in an excavation approximately 15ft. in depth, 30ft. in width, and 35ft. in length are not protected by an adequate protective system when operating the American Auger boring machine, when welding and cutting casings and when performing other tasks associated with installing casings for a water line.    The Mike Enyart and Sons, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, Protection of Employees from loose rock and soil, which was contained in OSHA inspection number 315225110, citation number 1, item number 4b and was affirmed as a final order on December 13, 2011, with respect to a workplace located on Washington Street in Paden City, WV.
