29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person    Located in front of the single-family house at 9536 McCauley Road, West Chester, Ohio workers were excavating a trench for a pipeline. The operator swung the Deere excavator around and the knuckle of the machine made contact with the 120/240 volt, single phase overhead service conductor that was attached to the house. The operator of the excavator was exposed to electrocution hazards in that a inspection of the jobsite was not conducted by a competent person prior to the start of this activity.
