29 CFR 1910.138(b): The employer did not base selection of appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the tasks to be performed, conditions present, duration of use, and the hazards and potential hazards identified: On or about July 22, 2016, and in times prior thereto, Rolshear rolling machine operator was wearing gloves that could be caught in the rollers resulting in a possible amputation injury. The roller was located in the production area just outside of the break room.

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: On or about July 22, 2016, and in times prior thereto, Rolshear rolling machine was not guarded to prevent the operator from being caught in the rollers which could result in amputations and/or crushing injuries. The roller was located in the production area just outside of the break room.
