On or about 12 July 2019, and at times prior thereto, the tongue guard on a pedestal grinder exceeded 1/4-inch gap, exposing the workers using the grinder to the hazard of being struck by fragmented pieces from the disintegrating wheel.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. On 1 July 2019, at approximately 9:30 AM, an employee suffered an injury which resulted in the amputation of the left ring finger to the 1st knuckle. The amputation was not reported to the Occupational Safety and Health Administration until 2 July 2019 at approximately 1:30 PM CDT, or about 28 hours later.
