(a)  Straight Line Painting, at 3155 E. 104th Ave., Thornton, CO 80233; On and before July 26, 2016, the exposing employer failed to ensure that employees working on a man lift wore lanyards and an appropriate harness or other approved device.  Two employees were painting near the top gables and were not tied off.  Condition exposed employees working on the lift to fall hazards of greater than 25 feet as they painted the side of the construction project.
