29 CFR 1910.95(c)(1): A continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (n) was not instituted when employee noise exposures equaled or exceeded an 8 hour time-weighted average sound level (TWA) of 85 dBA: a) Assembly Area, on or about 8/2/16: An employee working as a Assembler was exposed to continuous noise at a level of 117.0% of the dose of the OSHA permissible exposure limit. The equivalent dBA for the 117.0% dose is approximately 91.13 dBA calculated as the 8 hour time weighted sound pressure level. The sampling was conducted on 8/2/16 for 458 minutes. Exposure calculations include a zero increment for the 22 minutes not sampled. Employee was wearing MSA Ear Muffs. b) Assembly Area, on or about 8/2/16: An employee working as a Assembler was exposed to continuous noise at a level of 118.9% of the dose of the OSHA permissible exposure limit. The equivalent dBA for the 118.9% dose is approximately 91.25 dBA calculated as the 8 hour time weighted sound pressure level. The sampling was conducted on 8/2/16 for 423 minutes. Exposure calculations include a zero increment for the 57 minutes not sampled. Employee was wearing E.A.R. Plugs.

29 CFR 1910.95(g)(6): New audiograms were not obtained annually for each employee exposed to noise at or above an 8 hour time weighted average of 85 dBA: a) CNC Area, on or about 8/2/16: Annual audiograms were not obtained for each employee exposed to noise at or above 85 dBA. An employee working as a CNC Operator was exposed to continuous noise at a level of 72.8% of the dose of the OSHA permissible exposure limit. The equivalent dBA for the 72.8% dose is approximately 87.71 dBA calculated as the 8 hour time weighted sound pressure level. The sampling was conducted on 8/2/16 for 427 minutes. Exposure calculations include a zero increment for the 53 minutes not sampled. Employee was wearing SAS Ear Muffs.
