29 CFR 1910.147(a)(3)(i): The employer failed to establish a program and utilize procedures for affixing appropriate lockout devices or tagout devices to energy isolating devices and to otherwise disable machines or equipment to prevent unexpected energization, startup or release of stored energy in order to prevent injury to employees. a) 640 Marshall Street, Milford, DE - On or about November 17, 2016, the employer failed to establish a Lockout/Tagout (LOTO) program and did not utilize procedures for affixing appropriate lockout or tagout devices, exposing employees to electrical, crushing and amputation hazards.
