29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: (Construction Reference 1926.103)  a) Employees used helmet-type supplied-air respirators while performing abrasive blasting of the aerator box atop the roof of the treatment building at Runyon Water Treatment Facility at 2 Old Water Works Road in Old Bridge NJ.  Each respirator user was not provided a medical evaluation prior to assignment to duties requiring its use.  Condition(s) were noted or about 7/27/2016.

29 CFR 1910.134(i)(7): Oil lubricated compressor(s) used to supply breathing air did not have a high-temperature or carbon monoxide alarm(s) or both:  a) Employees performed abrasive blasting of the aerator box atop the roof of the treatment building at Runyon Water Treatment Facility at 2 Old Water Works Road in Old Bridge NJ; where supplied air for blasting helmets was supplied by DOOSAN brand oil-cooled compressor that was not equipped with carbon monoxide alarm and/or high-temperature alarm, so as to serve as a suitable source of breathing air.   Condition(s) were noted or about 7/27/2016.

29 CFR 1910.134(k): The employer did not provide effective training to employees who are required to use respirators:  a) Employees used helmet-type supplied-air respirators while performing abrasive blasting of the aerator box atop the roof of the treatment building at Runyon Water Treatment Facility at 2 Old Water Works Road in Old Bridge NJ.  Training was not provided to each respirator user that established their knowledge of the information in 1910.134(k)(1)(I) - (vii).   Condition(s) were noted on or about 7/27/2016.

29 CFR 1926.52(d)(1): 29 CFR 1926.52(d)(1):  Sound levels exceeded the values in Table D-2 of this section and a continuing, effective hearing conservation program was not administered:  a) Employees performed abrasive blasting of painted metal surfaces of tanks and other equipment, where personal monitoring on 7/27/2016 measured exposure to noise at 1310% and 1133% Dose; equivalent to 8-four Time-weighted average of 108.5 dBA and 107.5 dBA respectively.  The employer did not implement a hearing conservation program for employees with noise exposure exceeding the values in Table D-2, to include training & audiometric testing, both initially and at least annually thereafter.  Condition(s) were noted on or about 7/27/2016.
