OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushed-by hazards created by the use of industrial storage racks which were not anchored to the floor: a) Warehouse areas including Warehouses E and F, 344 Kendall Road, Blairsville, PA 15717: Observed on or about July 26, 2016, industrial storage racks located throughout the facility, including Warehouses E and F, were not anchored to the floor. The industrial storage racks were at risk of tipping over while being loaded or unloaded with powered industrial trucks. Employees loading the industrial storage racks and employees working in the area of the industrial storage racks were exposed to crushed-by and struck-by hazards.

Employees working in Warehouse D were exposed to a fire hazard created by restricted access to the emergency exit door. b) Warehouse D, exit door to right of the maintenance desk, 344 Kendall Road, Blairsville, PA 15717: Observed on or about July 26, 2016, exit access was restricted by the placement of a potable fan in front of the exit door. Employees working in Warehouse D were exposed to a fire hazard created by restricted access to the emergency exit door.

The gap between the work rest and the abrasive wheel was approximately 3/4 of an inch. Employees using the bench grinder were exposed to a caught-by hazard. b) Warehouse B, near the charging station, 344 Kendall Road, Blairsville, PA 15717: Observed on or about July 26, 2016, the work rest on the left side of a Craftsman brand 8 inch bench grinder was not adjusted to within 1/8th of an inch from the abrasive wheel. The gap between the work rest and the abrasive wheel was approximately 1 inch. Employees using the bench grinder were exposed to a caught-by hazard.

Employees using the bench grinder were exposed to a fire hazard and struck-by hazards because the gap between the tongue guard and the abrasive wheel exceeded 0.25 inches. b) Building B, near the charging station, 344 Kendall Road, Blairsville, PA 15717: Observed on or about July 26, 2016, the gap between the tongue guard and the abrasive wheel on the rights side of a Craftsman brand eight inch bench grinder was approximately three quarters of an inch. Employees using the bench grinder were exposed to a fire hazard and struck-by hazards because the gap between the tongue guard and the abrasive wheel exceeded one 0.25 inches.

The air guns used compressed air that was set to approximately 109 pounds per square inch (p.s.i.). Employees using the air guns to dust off equipment and work surfaces were exposed to the hazard of embolism.

29 CFR 1910.305(g)(1)(iv)(D): Flexible cords and/or cables where attached to building surfaces: a) Warehouses E and B, 344 Kendall Road, Blairsville, PA 15717: Observed on or about July 26, 2016, two flexible cables (480 volt, 3 phase) that were run from Warehouse E to Warehouse B were attached to building surfaces at various points along their lengths. The two flexible cables were plugged into receptacles in Warehouse E one of which was labeled 'Hydraulic Low Carbon Wire Winder.' The flexible cables ran along the ceiling from Warehouse E to Warehouse B. In Warehouse B the flexible cables were wrapped around the wooden rafters near the ceiling and draped over industrial storage racks near the low carbon wire winder. Employees working in the Warehouses E and B were exposed to the hazard of fire and electric shock. b) Warehouse F, 344 Kendall Road, Blairsville, PA 15717: Observed on or about July 26, 2016, a flexible cord (120 Volt) in Warehouse F was attached to building surfaces and partially concealed behind a wall. The flexible cable was tied to and wrapped around metal conduit above a doorway. The flexible cord was being used to supply power to a B TEK brand electronic scale. Employees working in the Warehouse F were exposed to the hazard of fire and electric shock.
