29 CFR  1910.133(a)(3):  The employer did not ensure that each affected employee, who wore prescription lenses while engaged in operations that involved eye hazards, used protection that incorporated the prescription in its design, or wore eye protection over the prescription glasses or protective lenses with side shields:  a)  Throughout the site, located at 2643 McRae Highway, Eastman, GA, on or about May 28, 2025, the employer exposed employees to struck-by hazards, in that the employer failed to ensure employees who wore prescription glasses were protected from eye injuries by enforcing the use of eye protection over prescription glasses, or providing prescription safety glasses.

29 CFR  1910.147(c)(1):  The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  a)  Throughout the site, located at 2643 McRae Highway, Eastman, GA, on or about May 28, 2025, the employer exposed employees to struck-by and caught-in hazards, in that the employer failed to establish an energy control program for employees to utilize while performing maintenance on the equipment throughout the mill.

29 CFR  1910.212(a)(1):  One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)  On the Sorter, located at 2643 McRae Highway, Eastman, GA, on or about May 28, 2025, the employer exposed employees to caught-in and amputation hazards in that, employees were required to perform work near the drive chain and sprocket of the sorter conveyor that was unguarded.    b)  On the end of the conveyor between the "perfect" peeler and the sorter, located at 2643 McRae Highway, Eastman, GA, on or about May 28, 2025, the employer exposed employees to caught-in and amputation hazards in that, employees were required to perform work near an ingoing nip point that was unguarded.   c)  On the conveyor between the "perfect" peeler and the sorter, located at 2643 McRae Highway, Eastman, GA, on or about May 28, 2025, the employer exposed employees to caught-in and amputation hazards in that, employees were required to perform work near the drive chain and sprocket that was partially unguarded.
