29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about July 21, 2016, a plant laborer was exposed to machine hazards associated with rotating parts during the cleaning (servicing) of a Ross, Model HSD-50, Serial No. 113428, 500 gallon mixer machine, known as mixer #3. The employer did not ensure the plant laborer implemented energy control procedures by turning off the machine [per the 1910.147(d)(2) requirements], isolating the machine from the energy source(s) [(d)(3)], affixing a lockout device [(d)(4)(i)], and verifying de-energization and isolation had been accomplished prior to cleaning the machine [(d)(6)]. Energy Sources include, but are not limited to, electrical, pneumatic, and hydraulic power.
