29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: Employee was working in an approximately 6 foot deep, 6 foot wide excavation with vertical walls. Employee was not protected by an adequate protective system. H & M Backhoe Services Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.652(a)(1)) which was contained in OSHA inspection number 954625, citation number 1, item number 3 and was affirmed as a final order on March 4, 2014, with respect to a workplace located at South 33rd Street & Bond Avenue, East Saint Louis, IL 62207.
