On or about July 28, 2016, at the above addressed jobsite, an electrical cabinet on the fourth floor was not provided with a cover to protect against accidental contact, thereby exposing employees to electrical hazards.

29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites.   a. On or about July 28, 2016, at the above addressed jobsite, electrical receptacles located on the 4th floor were not provided with GFCI type receptacles or an assured equipment grounding program, thereby exposing employees to electrical hazards.

29 CFR 1926.451(g)(3)(iv) Vertical lifelines, independent support lines, and suspension ropes shall not be attached to each other, nor shall they be attached to or use the same point of anchorage, nor shall they be attached to the same point on the scaffold or personal fall arrest system.    a. On or about July 28, 2016, at the above addressed jobsite, employees conducting masonry operations from a suspended scaffold, greater than forty (40) feet above the ground, were exposed to fall hazards due to the lack of a independent lifeline.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a. On or about July 28, 2016,at the above addressed jobsite, employees performing masonry operations from the roof were exposed to falls, greater than forty (40) feet, created by the presence of unprotected sides.
