29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. (a) Roselle, Illinois, East wall, glass stop cutting operation, CTD Whirlwind semi-automatic up cut saw: on or about August 2, 2016, operators were exposed to pneumatic clamps caught-under hazards where their points-of-operation were not adequately safeguarded to prevent left and right sides horizontal access during operating cycle. Operators had their hands and fingers exposed to machine hazards likely to result in serious crushing injury. Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d). (b) Roselle, Illinois, main production and fabrication cutting operations, CTD Whirlwind semi-automatic up cut saw: on or about August 2, 2016, operators were exposed to pneumatic clamps caught-under hazards where their points-of-operation were not adequately safeguarded to prevent left and right sides horizontal access and top vertical access during operating cycle. 29 CFR 1910.242(a): Each employer shall be responsible for the safe condition of tools and equipment used by employees, including tools and equipment which may be furnished by employees. Roselle, Illinois, fabrication areas, cordless power drills: on or about August 2, 2016, provided equipment for use in fabricating tasks was not operating in the intended safe condition where portable batteries were taped to hand grips due to damaged battery attachment clips. Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
