On or about August 1, 2016, in the compounding area, the employees wore half mask and full face respirators provided by the employer. The employees were not medically evaluated or trained in a respirator program. The employer did not establish and implement a respirator program.

29 CFR 1910.138(b): The employer did not base the selection of the appropriate hand protection on an evaluation of the performance characteristics of the hand protection relative to the task(s) to be performed, conditions present, duration of use. a. On or about August 1, 2016, in the compounding area, the employer did not evaluate the performance of the gloves used by employees required to work with powdered caustic. Employees were provided thin nitrile gloves which did not provided adequate protection thereby exposing employee's skin to the hazards associated with chemicals such as, but not limited to, sodium hydroxide.
