29 CFR 1910.178(m)(1): Trucks were driven up to employees standing in front of benches or other fixed objects: A. On or about June 10, 2016, 90 Bidwell, West Lot: A Clark CMP 70L propane-powered industrial vehicle, #NO732, was in the process of moving an empty enclosure cradle supported by wooden blocks. A pedestrian employee, acting as a load spotter, was not in the proper position. As the forklift was moved closer to the cradle, the top edge of the mast hit the cradle and caused the center of the cradle to fall from the wooden blocks. The pedestrian employee's foot was caught under the enclosure cradle, resulting in serious injury.
