29 CFR 1910.134(k): 29 CFR 1910.134(k): The employer did not provide the basic information on respirators in accordance to Appendix D of this section to employees who wear respirators when not required by this section or by the employer to do so: Reclaim room; On or about 9/27/16, the employee working in the reclaim room wore an N95 respirator when emptying the reclaimed rubble into the vacuum separator. Training, nor appendix D was provided and the respirator in use had been altered by removing one of the straps that held it securely on the face of the employee.

29 CFR 1910.1020(e)(1)(i): When an employee or designated representative requested access to a record, the employer did not ensure that access was provided in a reasonable time, place, and manner: Machining and Lamination Departments; On or about 8/9/16, the employer had not provided the results of personal monitoring for graphite and noise exposure to affected employees, at employees' request. Four employees were sampled for graphite dust exposure on 1/15/16 and 1/26/16. Two employees were monitored for noise on 5/11/16. Two of the employees monitored asked for the results and were not provided them.
