29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    On or about August 3, 2016 at the job site where an employee was exposed to a fall hazard while riding in an aerial lift (SN: 0300146724) to install plywood to the building without having a harness with a lanyard attached to the basket.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  On or about August 3, 2016 at the job site where an employee was exposed to a fall hazard greater than 6 feet while installing plywood to the building without a fall protection system.

29 CFR 1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and a grasping device was not provided to assist employees in mounting and dismounting the ladder.      On or about August 3, 2016 at the worksite where employees were exposed to fall hazards when accessing the upper landing surface with a portable ladder that was not secured at the top or provided with a grasping device.
