Site: 272 West 86th Street New York NY On or about 8/3/16 a) Employees were exposed to being struck by hoisting equipment. The rope used to hoist materials was cut and torn fibers.

Site: 272 West 86th Street New York NY On or about 8/3/16 a) An employee hoisting bricks to the 4th floor to the ground floor was exposed to falls of approximately 40 feet. The employee was not provided with fall protection.
