29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a)  On or about August 3, 2016, the employer, AJK Roofing, LLC (Martin Hernandez) exposed employees to injuries from falls, while conducting reroofing activities, without the benefit of fall protection systems being in place, on a roof in excess of six feet above the lower surface.      Martin Hernandez, doing business as MHM Roofing was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 641258, Citation Number 1, Item Number 1, and was affirmed as a final order on November 7, 2012, with respect to a workplace located at 1010 Moffat Circle, Colorado Springs, CO.    Martin Hernandez, doing business as MHM Roofing was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 463839, Citation Number 1, Item Number 1, and was affirmed as a final order on June 22, 2012, with respect to a workplace located at 3665 West Monmouth Avenue, Littleton, CO.
