29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about October 20, 2016 and at times prior, employees were exposed to approximately 10 to 15 feet fall hazards while working on the roof of a two-story residential house. The employees were not provided with a fall protection system while working on the roof.  Martin Hernandez, registered agent of AJK Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1166989, citation number 1, item number 1 and was affirmed as a final order on Sept 7, 2016, with respect to a workplace located at 3905 Oberding Dr., Colorado Springs, CO 80911.  Martin Hernandez, registered agent of MHM Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 641258, citation number 1, item number 1 and was affirmed as a final order on November 7, 2012, with respect to a workplace located at 1010 Moffat Circle, Colorado Springs, CO 80915.  Martin Hernandez, registered agent of MHM Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 701898, citation number 1, item number 1 and was affirmed as a final order on November 7, 2012, with respect to a workplace located at 6139 Treeledge Drive, Colorado Springs, CO 80918.  Martin Hernandez, registered agent of MHM Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 463839, citation number 1, item number 1 and was affirmed as a final order on June 22, 2012, with respect to a workplace located at 3665 West Mammoth Ave., Littleton, CO 80123.  Martin Hernandez, registered agent of MHM Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 98450, citation number 1, item number 1 and was affirmed as a final order on March 16, 2012, with respect to a workplace located at 7553 Manston, Colorado Springs, CO 80922.
