29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)    WORKSITE: The employer did not ensure that a competent person conducted frequent and regular inspections of the worksite, materials and equipment prior to employees torch cutting and removing pipes that had not been properly disconnected from the roof trusses.       On or about July 19, 2016, while employees were pulling trusses from the building, a fire sprinkler pipe that was not completely separated from the trusses struck a scissors lift. The scissors lift was knocked over causing an employee to sustained serious injuries from the incident.

29 CFR 1926.350(a)(9): Compressed gas cylinders were not secured in an upright position at all times:    WORKSITE: The free-standing oxygen and acetylene tanks in the following locations were subject to damage from vehicular traffics and equipment moving with in very close proximity, and they were not secured to prevent accidental displacement:       A.     Corner of the building next to a fuel tank and parking area in front of the garage door.  B.     Next to the wall on the opposite end of the fuel tank.
