29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:    On or about July 28, 2016, in the kitchen area, adequate working space between the cutting table and a electrical panel was not provided, in that the electrical panel was blocked, thereby exposing employees to an electrical hazard.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:    a. On or about 28, 2016, in the kitchen area, a Model H-600-DT Hobart dough mixer was hard-wired to an electrical outlet and did not have any strain-relief, in that both ends of the cord had exposed wiring. An employee attempted to move the mixer to another area of the kitchen and was electrically shocked by 190V of electricity, resulting in a hospitalization.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket):    a. On or about July 28, 2016, connected to a Model H-600-DT Hobart dough mixer, an electrical cable was not visibly inspected prior to being moved to a different area of the kitchen, in that the cable had no strain relief with protruding interior components, resulting in an employee being electrically shocked.
