29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. a. On or about July 29, 2016, at the above addressed jobsite, employees performing masonry operations from a supported scaffold approximately forty (40') feet above the ground, were exposed to fall hazards due to a lack of a complete guardrail system. The R. D. Masonry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(4)(i ), which was contained in OSHA inspection number 901878, citation number 1, item number 1 and was affirmed as a final order on June 20, 2013, with respect to a workplace located at 8125 West 171st St., Tinley Park, IL 60477.
