29 CFR 1910.23(b)(1)(i): Rail, roller, picket fence, half door or equivalent barrier was not provided on the opening for handling materials, the guards were not kept in position regardless of a door on the opening.  In addition there were not grab handles provided on each side of the opening with its center approximately 4 feet above floor level and of standard strength and mounting.  Building 3 mechanical room, Brockton VA Hospital, 940 Belmont Street, Brockton, Massachusetts; On or about August 3, 2016, employees were exposed to unprotected falls of approximately 12 feet while standing on the edge of the wall opening of the mechanical room while moving a pump and after the process was complete.

The Groen Direct-Steam Mixer Kettle did not have a cover to prevent employees from coming into contact with the hot water inside the kettle and within 2 inches from the rim of the kettle as the agitator rotated inside.  b.  The JET Milling/Drilling machine in the Welding Building did not have a point of operation guard to prevent employees from coming into contact with the bit during operation.
