29 CFR 1910.23(d)(1)(iv):  Stairways more than 44 inches wide but less than 88 inches wide was not equipped with one handrail on each enclosed side and one stair railing on each open side:  At the east entrance, the stairway (steps) did not have a handrail on its right side that ascended up to the door.  Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos, which you believe will be helpful.  The abatement certification sheet is enclosed with the notice(s).

29 CFR 1910.36(d)(1):   Employees were unable to open an exit route door from the inside at all times without keys, tools, or special knowledge.  A device such as a panic bar that locks only from the outside was not permitted on exit discharge doors:  The east entrance exit route door has a keyed lock only.  The hallway along this east entrance/exit route configuration is such that employees can get locked in between the east entry door and the north entry way.  An employee suffered  being locked in between the doors.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos, which you believe will be helpful.  The abatement certification sheet is enclosed with the notice(s).

In the Technician Cubicle, the Trip Lite Isotel 4-Ultra Relocatable Power Tap was series-connected with an extension cord that was connected to a floor outlet.   b.  In the Outreach Coordinator Cubicle, the Datagard Relocatable Power Tap was series-connected to an Isobar Ultra-4 Surge Suppressor that was connected to a floor outlet.
