29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person: (a)Gil Haugan Construction, Inc. @ 5001 West Delbridge Street, Sioux Falls, SD : On and prior to August 1, 2016, the exposing employer did not ensure a safety program was developed and implemented for the work environment to include regular inspections of the job-site, materials, and equipment; conducted by a competent person. On August 1, 2016, an employee was seriously injured after thirty-three precast panels collapsed due to inadequate support bracing and excessive wind load. This condition exposed employees to struck-by and crushing injuries.

29 CFR 1926.704(a): Precast concrete wall units, structural framing, and tilt-up wall panels shall be adequately supported to prevent overturning and to prevent collapse until permanent connections are completed. (a)Gil Haugan Construction, Inc. @ 5001 West Delbridge Street, Sioux Falls, SD : On and prior to August 1, 2016, the exposing employer did not ensure that precast concrete wall panels were adequately braced and supported to prevent overturning and collapse until the permanent connections were completed. On August 1, 2016,, an employee was seriously injured when thirty-three precast concrete panels collapsed due to an inadequate number of pipe tube braces installed on each panel and excessive eccentric lateral loads due to wind loading. This condition exposed employees to a struck-by and crushing hazard.
