On August 1, 2016,, an employee was seriously injured when thirty-three precast concrete panels collapsed due to an inadequate number of pipe tube braces installed on each panel and excessive eccentric lateral loads due to wind loading. This condition exposed employees to a struck-by and crushing hazard.
