29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: JOB SITE: 5337 Enchanted Drive, Weldon Spring, MO 63304, on or about 8/9/2016, workers were using an aluminum extension ladder to climb up and down the roof, the ladder's side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. The workers were exposed to a fall hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): JOB SITE: 5337 Enchanted Drive, Weldon Spring, MO 63304, on or about 8/9/2016, workers were observed on top a 8:12 pitch roof, approximately 9' 9" eave to ground level, actively removing old roofing material and were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measures. The workers were exposed to a fall hazard. G R Remodeling, Inc., formerly known as Javier Romero Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 685618, citation number 1, item number 1, issued on February 28, 2013 and was affirmed as a final order on March 25, 2013, with respect to a workplace located at 231 Imperial Lane, Fenton, MO 63026.
