29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents Location: 40 Washington Square South, New York, NY. Employees did not wear eye protection when removing a straight line that was installed on a masonry wall with two nails. One of the nails came loose and hit an employee in the left eye. Condition was noted on or about 08/10/2016.
