29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.    An employee assisting an operator on the set up of the 155 Panel Line machine had received training as an affected employee and had not received training on the recognition of hazardous energy sources, type and magnitude of the energy and the methods and means necessary for energy isolation and control of the 155 Panel Line machine idler and roller on the compression deck during the cleaning of the cardboard scrap from the idler.

29 CFR 1910.147(f)(1)(ii): Remove employees from the machine or equipment area in accordance with paragraph (e)(2) of this section;    On August 8, 2016, in the Production Area, on the 155 Panel Line Machine, an employee was exposed to a caught in-between hazard while removing cardboard material that had wrapped around the idler and the compression roller.

29 CFR 1910.253(b)(2)(ii): Inside of buildings, cylinders shall be stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. Cylinders should be stored in definitely assigned places away from elevators, stairs, or gangways. Assigned storage spaces shall be located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons. Cylinders shall not be kept in unventilated enclosures such as lockers and cupboards.    Next to the aisleway leading into the machines in the Production Area, two oxygen compressed gas cylinders were not located to where they would not be knocked over.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:    Employees who perform servicing and maintenance on the 155 Panel Line machine during cleaning of cardboard scrap on the compression deck idler and roller were not required to:    (1) know the type and magnitude of the energy sources, 480 Volt AC electrical energy source and 80 p.s.i. pneumatic energy source, the hazards of the energy sources to be controlled, and the method or means to control the energy,  (2) turn off and shut down the compression deck  (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine from the energy source  (4) affix lockout device to the energy isolating device  (5) prior to starting work on machines that have been locked out, verify that isolation and deenergization of the machine had been accomplished    Employees were exposed to an in-running nip point hazard while cleaning the idler and roller on the compression deck of cardboard scrap.      Honey Cell Inc. Midwest was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 110718, citation number 1, item number 1b and was affirmed as a final order on March 27, 2013, with respect to a workplace located at 6480 Hamilton Lebanon Road, Middletown, OH 45044.
