29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about August 9, 2016, and at times prior thereto, in the assembly area a horizontal band saw was used without blade guards provided to prevent contact with the operating blade. Employees were exposed to amputation hazards.

The employees were exposed to skin and eye injury.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On or about August 8, 2016, and at times prior thereto, in the laminated area a locatable power tap (RPT) was observed being used not in accordance with the listed instructions, including but not limited to the following instances: 1) In the laminated area a relocatable-power-tap (RPT) was used to provide power to equipment drawing large current. 2) In the laminated area a RPT was connected in "daisy chain" with an electrical extension cord. Employees were exposed to fire and electrical shock hazards.
