29 CFR 1910.217(b)(13): Control system(s) of mechanical power press(es) were not constructed so that a failure within the system would not prevent the normal stopping action from being applied to the press when required, but would prevent initiation of a successive stroke until the failure is corrected: (a) On or about August 3rd, 2016, a part revolution power press bearing the manufacturer's mark of "Federal 8 Elkhart Indiana" and bearing the company asset number of KC18, did, after completing a single cycle, descend from its top position unexpectedly causing serious injury to a press operator in the Fabrication Cell area of the plant. An accident occurred causing the amputation of two fingertips on the victim's right hand.

An accident occurred causing the amputation of two fingertips on the victim's right hand.

29 CFR 1910.217(f)(2): Supervision to ensure that correct operating procedures were being followed on mechanical power press(es) was inadequate: (a) On or about August 3, 2016, an operator of a part revolution OBI power press (company asset number KC18) in the Fabrication Cell area of the plant sustained serious injuries when the operator reached into the hazard area of the press to remove a part. The ram descended unexpectedly crushing two of the operator's fingertips. No tools were available to the operator for removing parts, and the operator had not been adequately trained or adequately supervised to avoid the hazard of reaching into the point of operation of a part revolution power press. Abatement certification is required for this item; use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.
