29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from potentially injurious UV light radiation:    a) Illing Compnay, Inc., Doraville, GA - In the production area, employees were exposed to irreversible eye damage when ultraviolet protective (UV) eye wear was not required or enforced for employees exposed to ultra violet (UV) lights from the Osmo Novax M-UV3 and  Kammann UV Bottle printing machines and which had protective shields removed or broken, on or about 08/10/2016.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):    a) Illing Company, Inc., Doraville, GA - Employees were observed operating two sit down, electric forklifts and two sit down propane forklifts, and had not complete the training and certification as required paragraph (l) of this standard, on or about 08/10/2016.    b) Illing Company, Inc., Doraville, GA - Employees were observed operating the Crown side shifter forklift and had not complete the training and certification as required in paragraph (l) of this standard, on or about 08/10/2016.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
