On or about August 12, 2016, and at times prior thereto, the employer did not ensure that electrical equipment was installed and used in accordance with any instructions included in the listing and labeling in three instances in the dry cleaning shop, exposing employees to the hazard of electrical shock and burns.     a. In the boiler room a six plug surge protector was being used as permanent wiring system.   b. In the boiler room an orange 3 plug receptacle was being used as permanent wiring system.   c. In the dry cleaning area behind the dry cleaning machine a six plug receptacle was being used as permanent wiring system.

In the boiler room the 240 volt conduit running to the boiler controls was broken and bent, the wire were exposed on the inside.  b. In the dry cleaning area on the wall behind the dry cleaning machines the wall was rotten and deteriorating and the electrical receptacle was loose.

In the stream area on the blue Ajax "Susie" Steamer an orange extension cord had been hard wired into the control box.  c. In the stream area on the yellow Ajax "Susie" Steamer an orange extension cord had been hard wired into the control box.
