29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:     The employer is failing to provide adequate employee training on the recognition of hazards specific to the work employees are assigned to perform prior to exposing the employees to those hazards.  The most recent occurrence of this was found at the facility located at 2702 W. 11th Street, Kearney, NE where the employer has staffed a temporary employee to an electrical contractor whom had the employee working on the construction of a new facility without having had adequate training.  Adequate training and instance of exposure include but are not limited to:   i.  Employees using an 8 foot fiberglass step ladder in a location where they can only reach while working blind overhead when they needed a 10 foot ladder so they could see into the box they were reaching into while they worked.    ii. Employees installing new wiring through existing conduit with live wiring already installed without considering the principles of Lockout/Tag Out by de-energizing the live wires prior to opening the junction box.
