29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    On or about August 3, 2016, and at times prior thereto, employees installing pipe couplings on 8-inch PVC sewer pipe in an unprotected greater than 5 ft. in depth trench were not wearing protective helmets to prevent struck-by hazards.

29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:    On or about August 3, 2016, and at times prior thereto, employees installing pipe couplings on an 8-inch PVC sewer pipe inside an unprotected trench greater than 5 foot in depth had no safe means of egress.

29 CFR 1926.652(b): The slopes and configurations of sloping and benching systems were not in accordance with the requirement options of 29 CFR 1926.652(b)(1), (b)(2), (b)(3), or (b)(4):    On or about August 3, 2016, and at times prior thereto, the employer had not designed the sloping and benching for a trench greater than 5 feet in depth for employees installing pipe couplings on an 8-inch PVC sewer pipe, including but not limited to allowable slopes, configurations for sloping and benching, designed by a registered professional engineer.  This condition exposed employees to cave-in hazards.
