(a)  On August 16, 2016, and at times prior thereto, atop the roof of the Yamaha Guam building,  a 6 foot shock-absorbing lanyard was attached to a handhold mounted on the exterior wall, exposing an employee to a fall hazard of about 14 feet to the asphalt below.
