29 CFR 1926.502(d)(15)(ii): Anchorages used for attachment of personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, and not installed under the supervision of a qualified person. (a) On August 16, 2016, and at times prior thereto, atop the roof of the Yamaha Guam building, a 6 foot shock-absorbing lanyard was attached to a handhold mounted on the exterior wall, exposing an employee to a fall hazard of about 14 feet to the asphalt below.
