29 CFR 1910.137(c)(2)(viii): Electrical protective equipment was not subjected to periodic electrical tests. Test voltages and the maximum interval between tests shall be in accordance with Table I-5 and Table I-6:     a) On or about August 17. 2016, at the above located workplace, employees required to work with 480 volt electricity used electrical insulating gloves.  The gloves were not subjected to periodic electrical tests in accordance with Table I-5 and Table I-6. Employees were thereby exposed to hazards associated with live electrical parts.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there was potential electrical hazards were not provided with, and use, electrical protective equipment that was appropriate for the specific parts of the body to be protected and for the work to be performed.         a) On or about August 17, 2016, at the above located work place, employees were required to perform voltage testing and working on 480 volt electrical equipment. The employer did not provide and require employees to use the following appropriate Arc-Flash Rated Protective Clothing:              1. Flame resistant (FR) clothing           2. Arc-rated face shield, or flash suit hood      Employees were thereby exposed to hazards associated with electricity and electrical explosions.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:   a) On or about August 13, 2016, at the above located work place, employees were required to operate and perform maintenance on 480 volt electrical equipment. The MPO 1-2, 480 volt disconnect which provided power to 1) Bake oven, 2) West Wall main, 3) Jetline welders, 4) 14 inch Seamer and 5) High bay machine, was not legibly marked to indicate its purpose, nor was it located so the purpose was evident.
