29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a.) Production - On or about February 12, 2021, the employer failed to provide adequate guarding to protect employees working at or near the Remy Hooper/Flanger, Model TNV720H, Serial No. 1/I, thereby exposing employees to amputations. While cycling the machine, an employee sustained three (3) partial finger amputations on the right hand.

29 CFR 1904.29(b)(1):29 CFR 1910.1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation: Skolnik Industries, Inc., Chicago, Illinois - On or about February 12, 2021, the employer failed to record the following workplace injury in the detail required by the regulation on the OSHA 300 Log for calendar year 2020. a.) On or about December 15, 2020, in the stainless steel bending area, a recordable injury occurred when a machine operator's right glove was caught in the roll bender machine, crushing the employee's right index finger. The information in Column F was not recorded in the detail required, as it did not describe the object that directly injured the employee.
