29 CFR 1926.451(e)(1): Where scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers or stairway-type ladders was not used for access: a) 639 E Townshipline Rd, Havertown, PA - The employer did not provide safe access for the employees to reach the working level of the ladder jack scaffold. The employees were working from a ladder jack scaffold platform, approximately 20 feet above the point of access. This hazard exposed the employees to a fall hazard on or about August 17, 2016.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) 639 E Townshipline Rd, Havertown, PA - The employer did not ensure that the employees were using some type of fall protection system when performing roofing operations on a steep roof thus exposing the employees to a fall hazard. This condition was observed on or about August 17, 2016.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level by use of personal fall arrest systems or guardrail systems: a) 639 E Townshipline Rd, Havertown, PA - The employees were working off of a ladder jack scaffold approximately 20 feet above the ground which did not have a fall protection system installed on the working level of the scaffold. This hazard exposed the employees to a fall hazard of approximately 20 feet. This condition was observed on August 17, 2016.
