Sub-contractor employees engaged in the installation of steel wall support rakers were not provided protection from falls with the secure installation of floor covers. Violation observed on or about August 15, 2016

Employees and Sub-contractor employees engaged in the rehab of a commercial property were not provided warning of floor holes by use of color coded or marked "Hole" or "Cover" on the wood covers.
