29 CFR 1910.37(b)(6): Each exit sign was not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color: a. On or about January 28, 2016 and at times prior, exit signs located at the north and south office exit doors and at the west side exit door loading dock area were not provided by a light source of at least five-foot candles. The exit signs would be difficult to see by the employees in the event of an emergency.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: a) On or about August 16, 2016 and at times prior, a flexible cord was used by the employees to provide power to a radio in the loading dock area. The flexible cord was not provided with strain relief.
