29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: On or about March 2, 2016, at the Jamul construction project, personal fall arrest systems were not inspected prior to use nor removed when defective. The snap hook on an employees lanyard had a defective gate that would not retract fully to the hook nose. This defective gate created an unsecured fall protection device and exposed the employee to a fall hazard.
