(a) At the Dickerson & Quinn warehouse, flexible cords were spliced and insulation were taped with regular electrical tape which was not of similar insulation properties. (b) The flexible cord used to extend power to the fan was spliced with two different type of cords and taped with regular electrical tape for insulation.
